International health Submit a declaration of health

The master of any vessel approaching the Port of Liverpool, or Port of Garston, from a foreign port must complete and submit a Maritime Declaration of Health.

Must all vessels submit a declaration?

Even in there are no suspected or confirmed cases of infectious diseases or death on board the vessel or aircraft, a Maritime Declaration of Health must be completed and submitted to us with all other ship arrival documents.

How do I submit a declaration?

A Maritime Declaration of Health must be submitted with along with all other ship arrival documents using our upload ship arrival documents form.

If there is an illness, or suspected illness onboard, the maritime declaration of health must be submitted separately from all other documentation. Find out how to report an illness on board.
